Sql Server - Unable to create new database due to lack of admin role

I can log onto Sql Server Management Studio without issues. I can't create a new DB however as it appears that I don't have permissions. I can't give myself more permissions I can't modify the SA login. I thought my default my windows BUILTIN account would have access. What am I missing? Thank you
