                          My device is Yu Yureka Plus running CyanogenMod 12.1 and Lollipop 5.1.1. There's an icon that looks like a square dot in the middle and two square brackets surrounding it like [[•]]. It's on the system icon side. It appears and goes automatically.

I tried looking for the icon on Google but couldn't find it.

How can I remove it and what is it?
