Black box, middle-top of the screen. What is this, what app is responsible for it, and how do I kill it?

The phone is an INOI A54 on Android 13, stock ROM, rooted with a recovery Magisk patch. There is this black bar in the middle of the screen, at the top, that appeared when I got a call.

After the call ended, the bar stayed and could not be removed. Upon clicking on it, it launches the dialer app via some "DIAL" intent/action. This bar is really annoying, and I would like to know what setting or app causes it to appear.
What I've tried already:
1) Looking through basically all notification settings
2) Uninstalling the dialer app
3) Changing dialer apps
4) Killing any processes I could find that are remotely related to the calling activity
5) Checked the accessibility settings, nothing useful there, too.
6) Also tried to search for "overlay" and "quick access" in the search bar, nope.
And I still could not fix it. The only "solution" is to reboot, but I'd have to do it every time someone calls me.
I think that something that can be tried is checking the UI layers in some kind of debugger and then searching by package. That would solve it, I think, but I don't know any.
So, what I'm asking is:
1) If this bar can be disabled by changing some setting, then what is it, and where do I find it?
2) If it can't, then what app/process causes it? So that I can kill/restart/disable it.

SMS Messenger launches voice calls with Google Voice, I want to launch with regular voice app

Cannot add contact from phone call history of Samsung Phone app

Opening some kind of notepad application is cumbersome both saving and retrieving: tap home, tap applications, find the application, start a new note, type number, end call, copy number, paste number into the dialer (how to do the last step is another question)

Yesterday I downloaded alternative applications from Fossify and set them as default ones.
Now I heared the phone vibrating. It seems that it rebooted without any obvious reason. After it booted, I found out that Phone, Contacts and Messages applications were resetted back to the Google's ones.
I thought that it was an automatic system update, but according to the log, the last system update was installed a month ago.
How to find out which application / service triggered the restoration? Was it Google Play? Was it some system guard service? Howwto find it out?

Even when the phone is locked to the maximum extent (bedtime on Google Family Link / Huawei parental controls), the child able to call friends. This is not what I want, especially during nights.
I have spent a lot of time looking for a solution. This seems like something easy to implement (even locking the whole dialer app), however it looks like one would have to root the device to achieve this.
I found one trail, *MMGuardian*, that claims to allow contacts white list, but this is paid software and I'm looking for a free solution.
There is a Google topic about this, on which a lot of people voted.
How come such a seemingly obvious functionality is not available?

I have a Motorola Moto E (2020) (model number XT2052-1) running Android 10 (build number QPGS30.82-141-17).
Recently, it started ending incoming calls as soon as I answer them: when the phone is called, it rings and displays the associated on-screen prompt to answer/decline the call, but when I answer it, the call immediately ends (i.e., the phone hangs up automatically). This has happened multiple times with multiple callers; at this point I'm unable to successfully answer any incoming calls. From the caller's side, it seems like the call is answered but then immediately disconnected (they're not put through to my voicemail).
I was able to answer an incoming call about a week ago and I haven't made any changes to my phone's configuration in months (I haven't installed any new apps, changed any settings, etc.).
I'm able to make outgoing calls with no issues.
I went through Motorola's troubleshooter for both *Calls are dropping* and *Unable to make or receive calls* (under the general *Calls* category) and also tried some potential solutions I found online.
At this point, I've tried:
* Clearing the phone app's cache and storage.
* Powering off the phone and then powering it on again.
* Restarting the phone.
* Force rebooting the phone.
* Restarting the phone in safe mode. (This apparently rules out the possibility of another app interfering with phone calls.)
* Checked for a software update (my phone is already running the latest version of Android available to it).
* Testing the proximity sensor (this seems irrelevant but was listed in one of Motorola's troubleshooters).
* Ensuring that the "Power button ends calls" option is disabled (again, seems irrelevant, but was in a troubleshooter).
I *haven't* tried a factory reset (yet).
I've found a lot of reports of this problem or similar ones occurring for phones running Android, but the cause is often a third-party app (which I seem to have ruled out by trying to answer an incoming call in safe mode).
I'd appreciate any suggestions of other things to try before performing a factory reset. I'm also concerned that, even if I'm able to fix this problem, it may "randomly" occur again in the future (which I'd only realize when I'm unable to answer an incoming call), so any thoughts as to what the root cause might be (software, hardware, an issue with my cell service provider) would be welcome.

I reverted the system version to 8.0, and it was working normally until recently.

So I want to disable/delete/edit the "Emergency Call" number. Is there a way to do this? I know about Call Confirm apps, but I don't want to use it because that would enable "Confirm" for every number - which I don't want. I don't pocket-dial any number except this one. And it happens 5 times a day.

I bought the phone and it should NOT be associated or managed by a third party nor controlled remotely in any way. Also, I have not set the developer mode.
I don't recall having seen this type of prompt before, is it a bonafide Android OS prompt?
Below I post the screenshots of the Phone app permissions and the prompt with the message "Unlock device microphone This unlocks access for all apps and services allowed to use your microphone" with the Cancel and Unlock possible actions.

Worse, if the phone call requires tapping numbers (e.g., a call center process), the heads-up notification must be dismissed before any dialer actions can be taken. Insofar as I can tell, the heads-up notification has no practical value as it does nothing that the dialer does not already do. *Note: This only occurs on incoming calls. The notification doesn't happen with outgoing calls.* Is there any way to disable this notification? It only appears during active phone calls. If the other party hangs up first, it disappears along with the dialer interface. Here's a screenshot. The heads-up notification is the white block at the top with the "Ignore" and "Answer" buttons.
